Thrombosis of the superior mesenteric and portal veins after splenectomy and/or distal splenorenal shunts.

چکیده
During the past ten years, 180 patients with liver cirrhosis and hematologic disorders underwent splenectomy and distal splenorenal shunts, 56 of whom were evaluated by CT, ultrasonography (US) and/or angiography, postoperatively at Hiroshima University Hospital and Hiroshima Red Cross Hospital. Five patients were asymptomatic; thrombosis was observed in 10; and two died of liver failure. Three patients each complained of postprandial epigastralgia with diarrhea, fever, and general malaise. Angiography was performed to evaluate the patency of the shuts; CT and US, to follow up the liver cirrhosis not the portal vein thrombosis. Since thrombosis of the portal venous system occurs frequently during surgery, and since it causes marked changes in portal vein hemodynamics, the portal and superior mesenteric veins must be observed by US and CT for the early postoperative detection and treatment, to avoid major surgery.

Fifty Years of Surgery for Portal
Background Data A variety of procedures have been used to treat bleeding varices during the past 50 years. These include transesophageal ligation of varices or devascularization of the esophagus and stomach with splenectomy; portal-systemic (total) shunts; distal splenorenal (selective) shunts; endoscopic sclerotherapy; transjugular intrahepatic portal-systemic shunts; and liver transplantation.
متن کاملRenoportal anastomosis in living donor liver transplantation with prior proximal splenorenal shunt
For transplant surgeons, end-stage liver disease with portal venous thrombosis and a previous splenorenal shunt (SRS) is a significant challenge during liver transplantation. Thrombosis of the portal vein can be corrected by surgical interventions, such as portal venous thrombectomy or surgical removal of the thrombosed portal vein. Even also placement of a graft between the mesenteric vein and...
متن کاملSize of gastroesophageal varices: its behavior after the surgical treatment of portal hypertension.
The size of gastroesophageal varices is one of the most important factors leading to hemorrhage related to portal hypertension. An endoscopic evaluation of the size of gastroesophageal varices before and after different operations for portal hypertension was performed in 73 patients with schistosomiasis, as part of a randomized trial: proximal splenorenal shunt (PSS n=24), distal splenorenal sh...
متن کاملPerioperative management of spontaneous splenorenal shunts in orthotopic liver transplant patients.
OBJECTIVES Spontaneous splenorenal shunts cause significant vascular steal from the liver. There is no accepted algorithm for treating spontaneous splenorenal shunts before, during, or after liver transplant, and evidence for efficacy of treatments remains limited. MATERIALS AND METHODS We reviewed the literature, and our institution's experience regarding spontaneous splenorenal shunts, incl...
متن کاملManagement of Isolated Gastric Varices by Devascularization and Proximal Gastrectomy in Cirrhotic Patients
Twenty patients with liver cirrhosis were treated by surgery for bleeding from isolated gastric varices. The presence of tortuous and engorged gastric veins connecting with a large splenorenal shunt was demonstrated by transhepatic portography in all patients. The surgical procedures consisted of splenectomy, proximal gastrectomy, paragastric devascularization, and ligation of the splenorenal s...
